How To Use MyHealth Analysis

The more information you give your doctor, the more information he has to think with and study your health record prior to your consultation. The information you enter in these forms is completely confidential and is not shared with any other source, unless you request this in writing. Our complete privacy policy can be viewed at this link. If you have problems completing your profile, you can contact your assigned on-line doctor / clinician at {doc_phone} or email us at the Customer Support Department.

Follow These Steps:

Your health profile contains several steps that need to be completed, depending on the number of physical complaints you have. In the table below, you'll be able to go to each of these screens and complete your health history. If you history has changed, your online doctor will enable you to create a new profile, or, if you are using MyHealth Record to keep accurate medical records you can request that a new profile be initiated. Your old profile will be saved as a permanent record and can be retrieved later. (See the Archived Medical Records page).
